You can be insured by Försäkringskassan and still not be entitled to subsidised public healthcare. Sweden runs two separate systems. Socialförsäkring (social insurance) is run by the state agency Försäkringskassan and pays cash benefits. Hälso- och sjukvård (healthcare) is run by the regions and provides care. Being in one does not put you in the other.
This guide covers the first system: who is insured, what its main benefits pay, and where healthcare starts.
Who this guide is for, and who it is not for
This is for anyone living or working in Sweden, and especially for partners and families who need to know what the household can claim. If you hold a permit and want to see what applies at each stage of residency, continue to insurance in Sweden by permit type and residency stage.
Two groups have a more specific guide:
- Students on a study permit deciding whether to buy a private health policy. Go to health insurance for a student residence permit and the one-year threshold.
- Visitors on a short trip. This guide describes people who live or work in Sweden, not visitors. Start at planning a trip to Sweden.
Three things that shape what you can claim
You do not apply to become insured, but three things shape what you can claim:
- Where you live and work. Försäkringskassan: "du behöver antingen bo eller jobba i Sverige. Det kallas för att vara försäkrad i Sverige" (you need to either live or work in Sweden; that is called being insured in Sweden).
- Whether you have a residence permit, if you need one. You "kan ha rätt till ersättning ... när du har fått uppehållstillstånd i Sverige. Däremot kan du oftast inte få ersättning medan du väntar på uppehållstillstånd" (you can have a right to benefits once you have a permit, but usually not while you wait for it).
- Whether you are folkbokförd. Folkbokföring is the registration that Skatteverket (the Tax Agency) handles. It is not a condition for being insured, but it is the usual condition for subsidised healthcare (see below).
Some benefits then need an application or a registration with Försäkringskassan, and each has its own conditions, such as an SGI, children, or income.
You do not need a personnummer. Försäkringskassan: "Nej, du behöver inte ha ett personnummer. Om du saknar personnummer ... kan du använda ditt samordningsnummer i stället" (no, you do not need a personnummer; if you lack one you can use your samordningsnummer instead). A samordningsnummer is an identity number from Skatteverket for people who do not have a personnummer.
What Försäkringskassan is
Försäkringskassan is the Swedish Social Insurance Agency. Its own description: "Försäkringskassans främsta uppdrag är att besluta om ekonomiskt stöd till barnfamiljer, sjuka och personer med funktionsnedsättning" (its main task is to decide on financial support for families with children, the sick and people with disabilities). It "ansvarar för den svenska socialförsäkringen, med undantag för pensioner", so pensions sit outside it. It also pays some benefits outside the core system, "till exempel arbetsmarknadsersättningar, tandvårdsersättning och bilstöd" (for example labour-market benefits, dental-care compensation and car support).
Försäkringskassan decides who is entitled and pays out. It is not an insurance company you buy from: "Till privata försäkringsbolag måste du själv betala för att vara försäkrad. ... Hos Försäkringskassan är du försäkrad genom att du bor eller arbetar i Sverige" (with private insurers you pay to be insured; with Försäkringskassan you are insured by living or working in Sweden).
Who is insured: residence or work
The law. Socialförsäkringsbalken (SFB, the Social Insurance Code, 2010:110) 4 kap. 2 § divides the system into three branches: "förmåner som grundas på bosättning i Sverige (bosättningsbaserade förmåner), ... förmåner som grundas på arbete i Sverige (arbetsbaserade förmåner), och ... förmåner som grundas på andra omständigheter än bosättning eller arbete i Sverige (övriga förmåner)". In English: residence-based, work-based, and other benefits.
The plain-language version. Försäkringskassan: "Ersättningar som man får i stället för att arbeta kräver oftast att du jobbar i Sverige. För Försäkringskassans andra ersättningar behöver du oftast bo i Sverige" (benefits that replace work income usually require you to work in Sweden; the others usually require you to live here).
| Bosättningsbaserad försäkring (residence-based) | Arbetsbaserad försäkring (work-based) | |
|---|---|---|
| What gets you in | You are bosatt (resident) in Sweden | You do förvärvsarbete (gainful work) in Sweden |
| Example benefits | Barnbidrag, bostadsbidrag (housing allowance) | Sjukpenning |
| Legal test | SFB 5 kap. 2 and 3 §§ | SFB 6 kap. 2 § |
Two details from the law. A person who arrives and can be expected to stay longer than a year is treated as resident: "Den som kommer till Sverige och kan antas komma att vistas här under längre tid än ett år ska anses vara bosatt här i landet" (SFB 5 kap. 3 §), unless there are "synnerliga skäl" (exceptional reasons) against it. From 1 January 2027 the general definition in 5 kap. 2 § adds lawful stay: a person counts as resident "om han eller hon har sitt egentliga hemvist här i landet och vistas här lagligen" (if their real home is here and they are lawfully staying here). Work means "förvärvsarbete i verksamhet här i landet" (gainful work in an activity here in the country; SFB 6 kap. 2 §).
Citizenship does not decide anything: "Du är inte automatiskt försäkrad i Sverige bara för att du är svensk medborgare. Det som avgör är i stället var du bor eller jobbar" (you are not automatically insured because you are Swedish; what decides is where you live or work). If you have moved to or from Sweden, or work abroad, you can be insured "för vissa ersättningar, men inte för alla" (for some benefits but not all), and Försäkringskassan checks this first whenever you apply. EU, EEA, Swiss and UK residents have special rules. Försäkringskassan: "Huvudregeln är att du är försäkrad i det land där du arbetar" (the main rule is that you are insured in the country where you work). The law draws the consequence in SFB 4 kap. 5 §: if EU Regulation 883/2004 puts you under another country's legislation, you are not insured in Sweden for the matching benefits. This is the case to check if you are posted here by an employer abroad, or live in Sweden and work in another EU country.
What the three main benefits pay
Försäkringskassan pays many benefits. These three are the ones households meet first. For each, the conditions below come from Försäkringskassan's own pages.
| Benefit | Who gets it | What it pays |
|---|---|---|
| Barnbidrag (child benefit) | A custodian (vårdnadshavare) of a child under 16, or older if the child is still in grundskola (compulsory school) or anpassad skola (adapted school), when both custodian and child are insured in Sweden and the child lives in Sweden, the EU/EEA, Switzerland or the UK | 1 250 kronor a month per child, split between custodians; a flerbarnstillägg (multiple-child supplement) starts at 150 kronor for 2 children |
| Föräldrapenning (parental benefit) | A parent who cares for the child, where the child lives in Sweden, another EU or EEA country, or Switzerland, and the parent is insured in Sweden | 480 days per child to share: 390 days at sjukpenning level and 90 days at 180 kronor a day |
| Sjukpenning (sickness benefit, for employees) | An employee who cannot work as usual because of illness or injury, whose work ability is reduced by at least a quarter, who loses income and who has an SGI | Paid from the 15th sick day; the employer usually pays during the first 14 days |
Barnbidrag. Parents "som bor och har barn i Sverige får barnbidrag automatiskt varje månad" (who live and have children in Sweden get barnbidrag automatically each month). If you move to Sweden with a child under 16, you must give Försäkringskassan your details so the benefit can be set up. The amount is "1 250 kronor i månaden per barn och delas mellan vårdnadshavarna", and "det är ingen skatt på barnbidraget" (there is no tax on it). Försäkringskassan names barnbidrag among the benefits "som baseras på bosättning" (based on residence). For people who settle in Sweden from 1 January 2027 it is no longer automatic: see "What changes on 1 January 2027" below.
Föräldrapenning. Days sit at two levels. A full day at sjukpenning level pays "mellan 250 och 1 259 kronor" (between 250 and 1 259 kronor), and a full day at the lowest level pays 180 kronor. Most working parents are paid from their sjukpenninggrundande inkomst (SGI, the sickness-benefit income base). Försäkringskassan says most people with a regular income from work get it based on their SGI. To get this you need "en SGI som motsvarar en månadslön på minst 7 083 kronor, under åtminstone 240 dagar innan barnets beräknade födelse" (an SGI equal to a monthly salary of at least 7 083 kronor for at least 240 days before the expected birth). A parent with no income still gets paid: "Då får du det lägsta beloppet på sjukpenningnivå" (then you get the lowest amount at sjukpenning level). For a parent who settles in Sweden from 1 January 2027, föräldrapenning at the basic and lowest levels needs the qualifying condition described below. Försäkringskassan must have the application "senast 90 dagar efter första dagen du var hemma med barnet" (at the latest 90 days after the first day at home with the child). For a child born in 2016 or later, 90 days at sjukpenning level are reserved for each parent; for a child born in 2015 or earlier it is 60.
Sjukpenning. For employees: "De två första veckorna när du är sjuk betalar din arbetsgivare lön till dig. Sedan kan du ansöka om sjukpenning från Försäkringskassan" (for the first two weeks your employer pays you; then you can apply to Försäkringskassan). You can get it "från den 15:e dagen du är sjuk", but your employer must have reported your sick leave first. It can be paid at 25, 50, 75 or 100 percent, depending on how much your illness reduces your ability to work. Other situations, such as running your own company, are not covered here.
A partner who does not work can still be insured by living here. What they can claim depends on each benefit's own conditions, which Försäkringskassan lists on the benefit's page. From 1 January 2027 that includes a qualifying condition for newcomers, set out next.
What changes on 1 January 2027
A law passed in May 2026, Lag (2026:853) om ändring i socialförsäkringsbalken, changes two things from 1 January 2027.
Being resident will also require lawful stay. The new SFB 5 kap. 2 §: a person is resident in Sweden "om han eller hon har sitt egentliga hemvist här i landet och vistas här lagligen" (if their real home is here and they are lawfully staying here).
Newcomers must qualify for eight residence-based benefits. SFB 5 kap. 11 a §: these benefits "får lämnas till en person som bosätter sig i Sverige endast om han eller hon uppfyller ett krav på kvalificering" (may be paid to a person who settles in Sweden only if they meet a qualifying condition), either by having "varit bosatt i Sverige under fem år" (been resident in Sweden for five years) or by having "en viss inkomst av förvärvsarbete" (a certain income from work).
- The benefits (11 g §): föräldrapenning at grundnivå and lägstanivå (the basic and lowest levels), barnbidrag including flerbarnstillägg, bostadsbidrag (housing allowance), omvårdnadsbidrag (care allowance for a child), sjukersättning and aktivitetsersättning paid as garantiersättning, merkostnadsersättning, äldreförsörjningsstöd and bostadstillägg. Föräldrapenning at sjukpenning level, paid on an SGI from work, is not on the list.
- Five years (11 b §): counted "inom en ramtid om femton år" (within a fifteen-year window) from the day the application concerns.
- Income (11 c and 11 e §§): at least 0.48 inkomstbasbelopp (income base amount) a month for at least six months in a row, or 0.25 inkomstbasbelopp a month for at least 12 of the last 24 months. Försäkringskassan puts these at "ungefär 40 000 kronor per månad minst 6 månader i följd" and "ungefär 21 000 kronor per månad under 12 av de senaste 24 månaderna". Income counts if employer contributions or egenavgifter (self-employed contributions) are due on it.
- Exceptions (19 §): the benefit can be paid without qualifying "Om det finns synnerliga skäl" (if there are exceptional reasons), and only then.
Who it hits. The transitional rules say "Äldre bestämmelser gäller för personer som bosatt sig i Sverige före ikraftträdandet" (the old rules apply to people who settled in Sweden before the law took effect), and Försäkringskassan: "Personer som är bosatta i Sverige före den 1 januari 2027 omfattas inte av de nya bestämmelserna." Residence periods before that date are judged under the old wording of 5 kap. 2 §, without the lawful-stay test. So the change hits people who settle in Sweden on or after 1 January 2027. A parent in that group who has no income from work does not get barnbidrag, or föräldrapenning at the basic and lowest levels, until the five-year or income condition is met.
Takeaway: if you move to Sweden in 2027 or later, do not plan your household budget around barnbidrag, low-level föräldrapenning or bostadsbidrag until you have either five years' residence or a work income at the levels above.
Healthcare is a different system
A worker with a samordningsnummer is insured by Försäkringskassan. Does that give subsidised public healthcare, or does that start only at folkbokföring?
Folkbokföring is what Försäkringskassan points to. On its page for people with a residence permit: "Måste jag vara försäkrad hos Försäkringskassan för att få vård om jag blir sjuk? Nej, för att få sjukvård i Sverige räcker det att du är folkbokförd hos Skatteverket" (must I be insured by Försäkringskassan to get care? No, being folkbokförd with Skatteverket is enough). On its page for people who work here: "Jag jobbar i Sverige. Kan jag gå till läkaren och tandläkaren i Sverige? Du som är folkbokförd i Sverige har rätt till sjukvård. Du betalar vanlig svensk patientavgift ..." (I work in Sweden; can I see a doctor and dentist? If you are folkbokförd you have the right to healthcare, and you pay the ordinary Swedish patient fee).
The law ties the duty to residence. The region's duty sits in hälso- och sjukvårdslagen (HSL, the Health and Medical Services Act, 2017:30) 8 kap. 1 §: "Regionen ska erbjuda en god hälso- och sjukvård åt den som är bosatt inom regionen" (the region must offer good healthcare to anyone resident in the region). One extension is in 8 kap. 2 §: care for people who, "utan att vara bosatt här" (without being resident here), have a right to care benefits under EU Regulation 883/2004. For EU, EEA, Swiss and UK citizens who are not folkbokförda, Försäkringskassan says they "kan få vård på samma villkor som de som är folkbokförda i Sverige" (can get care on the same terms), and "för att få vård behöver du visa upp ett intyg från Försäkringskassan" (you need to show a certificate from Försäkringskassan).
One region's practice. Region Stockholm, on 1177: "Även om du har fått uppehållstillstånd ger det dig inte rätt till subventionerad vård från Region Stockholm. ... För att du ska få subventionerad vård måste du bli folkbokförd här" (even with a residence permit you have no right to subsidised care from the region; to get it you must become folkbokförd). Check your own region's 1177 page for how it handles people who are not folkbokförda.
When do you become folkbokförd? Skatteverket: "Du som har flyttat till Sverige och planerar att bo här i minst ett år ska i de flesta fall vara folkbokförd här" (if you plan to live here at least a year you should in most cases be folkbokförd). If you plan a shorter stay, "kan du ansöka om samordningsnummer" (you can apply for a samordningsnummer). A worker on a stay under a year, with a samordningsnummer, may be insured for work-based benefits such as sjukpenning (which also needs an SGI) but may not be able to rely on subsidised care. Confirm with your region before you need a doctor. Whatever your status, the region must give immediate care if you need it: "Om någon som vistas inom regionen utan att vara bosatt där behöver omedelbar hälso- och sjukvård, ska regionen erbjuda sådan vård" (HSL 8 kap. 3 §). What changes is the fee.
Plan for that gap with how to see a doctor before your personnummer arrives and the overview of healthcare in Sweden.
Registering with Försäkringskassan and being folkbokförd are two separate steps with two separate agencies: "Om du ska jobba i Sverige men inte är folkbokförd här, så ska du anmäla dig till Försäkringskassan" (if you will work in Sweden but are not folkbokförd, you must register with Försäkringskassan).
Where private, home and car insurance fit
Försäkringskassan itself draws the line: "Till privata försäkringsbolag måste du själv betala för att vara försäkrad. Du kan till exempel betala för att försäkra ditt hem eller din bil" (with private insurers you pay yourself; for example to insure your home or your car). Private insurance is bought from a company and pays out under that company's policy terms, not as a state benefit.
- Hemförsäkring (home contents insurance) is one example of insuring "ditt hem". See what a basic hemförsäkring policy pays for and the guide for before you have a personnummer.
- Car insurance. The law requires trafikförsäkring. Trafikskadelagen (1975:1410) 2 §: "Trafikförsäkring skall finnas för motordrivet fordon som är registrerat i vägtrafikregistret och inte är avställt ... Försäkringsplikten fullgörs av fordonets ägare" (traffic insurance must exist for a motor vehicle that is registered and not deregistered; the owner fulfils the duty). The levels above that minimum are explained in trafikförsäkring, halvförsäkring and helförsäkring.
- Private health insurance and top-up cover. These are sold by insurers on their own conditions. See private health insurance in Sweden and top-up health insurance for what they cover.
If Försäkringskassan says no
If you disagree with a decision, the steps for asking for a review and appealing are in appealing a Försäkringskassan decision.
Common questions
I have a work permit and a samordningsnummer. Can I use a vårdcentral (health centre) at the normal fee? Försäkringskassan says folkbokföring is what gives the right to care, so check with your region first. Your permit length and the one-year rule are on Skatteverket's Flytta till Sverige page.
Does my child have to be insured too? For barnbidrag, yes: "Både du och barnet är försäkrade i Sverige". If you move to Sweden with a child under 16, you give Försäkringskassan your details so the benefit can be set up.
